2016 MassMATE Conference Registration

Please ensure that the Purchase Order lists the names of all the attendees. If you have more than 5 attendees, enter the first 5 and then return to this form to complete registration for the rest of the group. All attendees must be registered. If there is need for a substitution, requests can be made by e-mail before May 18, 2016. All participants must agree on the topics of their presentations with the conference organizers.
